Kidney Care Center Alliance was formed by Kidney Care Center, LLC, a Physician owned group practice as a KCE entity to participate in the Medicare value-based program known as Comprehensive Kidney Care Contracting Model (CKCC). The model is aimed at slowing the progression of CKD, increasing home dialysis penetration, promoting kidney transplant utilization, better dialysis starts and care coordination for Medicare beneficiaries with CKD stages 4 and 5 and ESRD, and improving quality of care and reducing the overall cost of care.

To enhance care coordination and expand care access to Medicare beneficiaries in this model, KCCA partners with Kidney Care Center providers, Transplant Center, and VillageHealth to provide a holistic approach to care coordination and management for better quality outcomes for our patients. The collaboration among these providers will bring about a greater level of care that increases home dialysis penetration and retention, delay the onset of dialysis for ESRD beneficiaries, promote kidney transplants, and improve overall quality outcomes.

In addition to the experienced care management team, participating beneficiaries will have access to relevant information and care resources to help and guide them through the course of their kidney care journey.
